Please complete the form below detailing your request and a member of our enrollment department will respond as soon as possible.
Please Note that the Tribal ID card will not be mailed until a money order ($20 per card) is received per resolution #32-1332-2025-02
Resolution #32-1332-2025-02 hereby approve the checklist and application requirement for IDs for off-reservation members and authorize the Enrollment Department to send IDs once those requirements are met, and all off-reservation IDs will be $20.
