20 Feb Tribal Identification Card Request Form (For Off Reservation Tribal Members)

Please complete the form below detailing your request and a member of our enrollment department will respond as soon as possible.

Please Note that the Tribal ID card will not be mailed until a money order ($20 per card)  is received per resolution #32-1332-2025-02

Resolution #32-1332-2025-02 hereby approve the checklist and application requirement for IDs for off-reservation members and authorize the Enrollment Department to send IDs once those requirements are met, and all off-reservation IDs will be $20.

 

Please make the money order out to the following and send to the same address.

The following is an example of how the money order should look. Please DO NOT sign the back of the money order, as that is for the finance office to sign.
Sign the part that says purchasers signature, which is usually in the lower right corner. If possible, please print your name in the memo.
